St. Luke's Heart Failure Clinic is the only heart failure clinic in the region with cardiologists who are board certified in advanced heart failure and transplant cardiology. Our team will work with you and your family to create an individualized plan of care that includes education about heart failure and management of your symptoms to help you better understand your condition and reduce hospital readmissions. We'll also identify ways to optimize your recovery or slow disease progression, including the opportunity to work with a registered dietitian to receive diet education and support.
We also coordinate care across various specialties, as needed, including home health, palliative care, and hospice care; and partner with Intermountain Medical Center in Utah to provide advanced heart failure therapies, such as transplant and left ventricular assist device (LVAD) surgeries. In addition, our financial advocate can advise you on health-related costs.
